ciprofloxacin 500 mg bid for 7 days, or

levofloxacin 750 mg qd for 5 days

- Complicated pyelonephritis

- Severe illness e.g., high fever, severe pain

- Inability to take oral fluids or medications

- Pregnancy

- Concerns about patient compliance
